طرابلس المحاصرة
الوضع في طرابلس الاسيرة اصبح روتين من اطلاق النار من قبل الكتائبالمنية المتواجدة في كل نهاية شارع تقريبا حتي اخر ساعتين قبل خروجي شهدت اعتقال احد الشباب ومصادرة سيارته من قبل الكتائب والغريب ان حوالي سبع سيارات قامت بعملية الاعتقال وكأنها تعتقل قائد عسكري وليس مجرد شاب خارج من محل مواد غذائية
الكتائب عاثت في العاصمة فسادا فقامو بمداهمة بيت احد الجيران وسرقة اجهزة التلفزيون وبعض الاغراض الاخري ، اصبحو يتمتعون بأوامر المداهمة لانها تصريح بالسرقة والنهب وهتك الاعراض كيف للانسان العيش هناك وهو لا يدري هل سيموت بطلقة طائشة من احد افراد الكتائب السكاري او ان يقتل في محطة بنزين لاعتراضه علي الفوضي التي يقوم بها افراد الكتائب المسيطرين علي محطات البنزين.
طوابير السيارات عند محطات البنزين تصل حتي عشرة كيلومترات في بعض المناطق اي خمس او ست ايام لتصل الي المحطة.
عند سماع صوت الطائرات الكل يخرج الي سطح المنازل والفرحة تعم عند سماع القصف واصوات التكبير ترتفع في القصف الليلي لطائرات الناتو مع التصفير واللزغاريد في بعض الاحياء
الكتابة علي الجدان كل يوم وفي كل مكان، الشباب يحاول عدم الكتابة علي البيوت كي لا يتضرر اصحابها لذا اصبحت المدارس والمقابر والاسواق كلوحات جدارية لجميع الشعارات الرافضة للنظام
علم الاستقلال يرفرف كل يوم في احدي مناطق طرابلس واكاد اجزم انه منذ شهرين لم يخلو يوم لم يرفع فيه العلم في مكان ما في طرابلس

Jwaji ( mood )
Yes I am still alive and I have no reason to leave my blog like this for this long time , I am as busy as everyone of you so the reason I guess is that I don't have the mood or what we call in Libya Jwaji to log into the blog world .
speaking of moods or jwaji as I prefer to call it, I want to say how much this word means to us as libyans , everything in here is driven by jwaji , why this street is closed for constructions today without any warning ? it's simple somebody's jwaji told him to close it and work here today , why the guy working in this office not here doing his job? ,well we are sorry he doesn't have jwaji to work today , in TMC ( tripoli medical center ) they closed the gate to the main parking area and now you have to take a whole big turn around Libya to get to the parking area and when I asked one of the security staff working there and insisted on knowing why he just laughed and said jwaji, everywhere you go you would find somebody working according to his jwaji so my advice to you next time you go to a place where you need to be served you ask the man or the lady behind the desk do you have jwaji today if yes then it's your lucky day if not then try again some other day when they have jwaji for you.

Thumb, Laptop and Libya
So what happened with me all this time I was away?
- I had a deep cut wound at the base of my left thumb, it was deep enough to cut the tendon causing me to loose the movement of the finger at the distal joint, I had a minor operation on it where they connected the tendon ends to each other but the movement is still not that good yet it needs time and physiotherapy, I only removed the stitches yesterday after three weeks of having my hand in a cast, inshallah I regain the full movement of the finger soon (please id3oli )
- I got a new laptop (((( FINALLY )))) HP pavilion 6426us so no more waiting for the computer to be free.
- I finished my Medicine course and this week is the last weel in Orthopedics course which I didn't get much done in it because my hand was splinted and when every patient would see me they would stare and think of the Libyan saying ( ما لقي الطب لروحه )
- One of my best friends came back from Greece to stay here forever so everyone is bothering him with WHY DID YOU COME BACK HERE??!! like him I faced this question so many times before, I can never think of leaving Libya forever, there are things here you wouldn't find anywhere else like not getting a speed ticket no matter how fast you drive :P and a very long list of things you find here that makes us live outside this world
- somehow I forgot to share with you all how I injured my thumb, I was on a ladder picking something from an aluminum shelve (scafali) the ladder slipped away and while I was falling involuntary as any human being would do I grabbed the edge of the shelve and you know the rest. There was blood everywhere, for the first time I saw my finger wide open. Trust me, it was not a pleasent sight. I quickly ran to the bathroom and put some water on the cut and checked it out to find that it's really deep.. I tried moving the finger but it wouldn't move :(
- I broke my glasses which I had for more than 3 years (a new record) so I had to buy new frames. Buying glasses is like getting a new face, they have to be nice enough because I am gonna see myself wearing them everyday and other people will see me wearing them. So I went to my favorite shop and bought these half nice half girly glasses I seriously do not know how the girl in the shop convinced me to buy them, I think the part where she said they make me look thin made me decide to buy them immediatly. However, only after buying them I starting thinking in what way exactly do they make me look thin? Us men get fooled easily by women thats why women shouldn't be allowed to work in places where men shop. Some people take advantage of our weakness and put a girl to answer the phone for customer service. Because if the caller was a guy and a nice friendly women voice is on the other line, that voice would make him forget why he called in the first place and if it was a women they would end up talking for hours on the phone about something else. With women you can never win, its either you back out or loose :P

Libya and medical ethics
It's just so sad to see a doctor with no ethics and to see a patient treated as a subject not as a human being , private hospital or public it doesn't matter because the same mentality working in either places, that what makes almost 90% of the Libyans go to Tunisia for therapy or check up , even most of the hospitals in Tunisia are working only to get as much money as they can from the Libyans but I can't blame the people who go there because I know at least they will get the attention and the psychological support that they will never find here, and it's not only our doctors and health staff who make all these people seek health care outside Libya but also the shortage of everything in our hospitals starting from the plaster to the Imaging machines .

When it was decided that I will write the editorial for the second issue of the Libyan Journal of Medicine, I was not sure where to start. There are many topics that are related to global health issues and many are specifically related to the health system in Libya. For this issue, I decided to start some discussion on medical ethics as it relates to the Libyan medical culture. In our culture we tend not to inform the patient about the seriousness of his or her illness especially if the case is terminal. We do tell the family and some times other relatives. We even answer questions about it during social events. This means that the patient’s lack of information is complicated by lack of privacy and confidentiality. We have very little time to sit with the patient and discuss the issues with him. Of course the situation is better in private practice (anecdotal, but no scientific studies). Some consultants tend to make rounds as they wish without respect to their teaching duties or their patients needs. Others have neglected patients after a major surgical procedure and not visited the patient at all until the patient’s death. Is it a wrongful death? The answer is beyond the scope of this editorial. It is not unusual to be sarcastic and make fun of the older patient who present with symptom of sexual dysfunction and remind him that he is too old for that and he should be ashamed of himself. The social status of the patient is a key factor for how he or she will be talked too and treated. If you are rich or poor, high ranking official or average human being, young person or old, presented to a Government hospital or to a private clinic, all of the above criteria may make a difference in how the patient is received or treated.
I have to admit that the issue is very complex. Most of the ethical or unethical behavior of physicians of my generation was obtained as part of the cultural and the religious backgrounds. No courses on medical humanity and ethics were taught as part of medical school curriculum. One might argue that common sense and the basic principles of our religion should be adequate to lead us to the ethical behavior. That might be true to some extent, but when you see your teacher’s behavior over seven years in medical school, and during the time you spend with him or her (attachment, internship, house officer) you will certainly be influenced. Therefore, it becomes our responsibility to read about medical ethics and to develop the correct courses with the help of professionals in the field.
It is our obligation to be the role models for the new generation of physicians and medical professionals. It is our obligation to show our students and residents that the patient is not a number. The patient is a human being who is suffering from illness, and came to you as an expert in the field to solve his problem. If we learn to show them that, then we will also show them that we need to talk to the patient with respect, honesty and full truth. We need to listen to them and give them a chance to explain all their symptoms. We show the students how we take patients’ complaints seriously. It does not matter if the case is in a general hospital or a private clinic. We need to go back to a very basic principle which tells us that if you lack knowledge you cannot teach it. So if you lack the ethical behavior, then you cannot teach medical ethics. I am stressing our role as seniors to start with ourselves and improve our ethical behavior. We also need to make sure that we have a strong course of medical humanity and medical ethics being taught at our medical schools.
It makes sense to me if all medical schools in the country have one committee to develop such curriculum to be taught cross the country. There is a need for a code of ethics at all levels. We need Code of ethics for the secretary of health, faculties of medicine and all hospitals. We need to train and form ethics committees in all hospitals and teaching institutions.
As Libyan Journal of Medicine being the official publication of Ibnosina Medical Association, it makes sense that we follow Ibnosina (Avicenna) footsteps. He was the ideal physicians and philosopher who followed ethical principles with his profession. For that reason UNESCO established an Ibnosina prize for ethics in science during its 166th session (2002). The first prize was awarded to Professor Margaret A. Somerville, Professor of Law and medicine at McGill University, Montreal (Canada). I believe it is about time to make medical ethics part of our life in Libyan medical education and profession. Maybe one day we can nominate one of our professors to this prize. At this point I think it would be a challenging task to choose one.

any way here is some pctures from the exhibition dedecated to misratia who couldn't coem this year .
LTT was the first thing I saw just to the right of the entrance , I asked them some questions but again there were no useful answers , but at least they said may be this September they are going to lower the prices for the adsl to 20 dinars instead of 50 a month and also make some changes on the services to make it better than it's now .
LTT had open WiFi Internet connection in the area of the exhibition, and they also had this questionnaire that I answered quite honestly so I hope they don't cut my connection at home .

في يوم كانت الشمس ما شاء الله تحرق وفي عز القايلة مع الساعة واحد , وقفت اول تاكسي وما قعدتش نستني في السيارة اللي تعجبني زي العادة , الحمد لله اول ما ركبت حسيت بظل السيارة لكن اول ما حاولت اني نزل مرش السيارة ما باش ينزل , السواق ابتسم وقاللي معليشي توا كيف فسد اليومين اللي فاتو بس , باهي اني قلت تو السيارة تتحرك و الهوا يخش من روشنه هوا ويخبط وجهي لكن المصيبة الطريق كانت زحمة عل الاخر طبعا لان فيه ثانوية بنات في نهاية الشارع وما نحكيلكش علي الشباب اللي زي الورد اللي ماد وجهه زي مالقي العزي ومتجمهر تقول قدام ملعب كورة, توا نبي نعرف شني فييه جاي ماد وجهه يتفرج علي والله قمة الوقاحة والصرصرة و المشكلة انه فيه وحدين تشوفه تقول نديد بوي , والله ما عرفتش شني الحل لهذي المشكلة كل يوم نفس الشي حتي والله لو بنوصل امي ما نخطمش من غادي باش ما نحطش روحي في موقف ونشبح واحدين يجرو في جرة واحدة يعاكسو فيها , شبابنا قاعد متخلف لدرجة ما يتصورهاش العقل , المشوكةونظارات الدبانة والسروال المشرك وتلقي شعر وجهه قاعد ما نبتش لكن السبسي في فمه داير دخاخين تقول مردومة, وكالعادة اللي يناشب يناشب كلام يوقف الشعر و اللي يسطرب يسطرب واللي فاتح شريط زمزامات واللي فاتح فيفتي سنت ( شباب متعدد الثقافات ماشاء الله) , تي والله لو عندي فيهم حكم كان جريدة خضرا وتمشليط لين يولي مخطط زي حمار الوحش باش وين يشبح ثانوية بنات يحط عيونه علي كندرته ويفوت جري .
السواق كان يشبح لساعته واحنا قاعدين في الزحمة و مرة مرة يشبحلي كيف نغلي من الحرارة ومن اللي صاير برة , ممكن قاعد يعدلي قداش دقيقة بنتحمل ولا شني , المهم طلعنا من الزحمة و شبحلي و ورالي سنونه كلهم وقالي معليشي انا عندي مشوار ضروري وخايف نتأخر لو وصلتك , جت منك يا جامع, انا مخنوق اصلا من الحرارة قلتله باهي نزلني وما دفعتش شي طبعا , توا قلت لروحي ما نبيش الحصلة الاولي ومش حنوقف غير التاكسي اللي يعجبني زي العادة , شبحت شيفروليت جديدة تلمع قلت تمام ممية مية , الصراحة انا كنت نبي نجرب الروشن قبل ما نركب بس خفت صاحب التاكسي يفهمني غلط علي العموم السيارة قاعدة جدبيدة وقلت بسم الله وركبت , وزي المرة الاولي والله حطيت صبعي علي البطمة الروشن ما باش ينزل شبحت للسواق وانبقيت بالضحك, قعد يضحك حتي هوا لكن مش فاهم اللي صاير, قاللي خيرك , قلتله عالي صار معاي و حكاية الروشن , قاللي الباب كيف مركبه جديد ومش عارف الروشن شوره مش معدل , عادي لو تبي تنزل ما فيش مشكلة ,لكن انا قلت يلا فيها خيرة خلي نكملو معاه اصلا انا وليت عرق وليت .
مرة ثانية بعد صلاة المغرب طلعت من الجامع و صاحب التاكسي كيف طلع حتي هوا ركبت معاه , حط شريط قران ما شاء الله الشيخ الدوكالي, بعد خمسة دقايق شلوب طلع الشريط و شلوب دخل شريط تاني أطبطب وادلع علي طول رقبته .

Yesterday was the big game here in Libya , Al ahli vs Al Iti7ad , I can't say that I am Al ahli fan but I know I don't like AL iti7ad at all , so I went to the game yesterday with my friends and it was a hell of experience , first the crowd outside was outstanding and unbelievable especially the police was closing all the doors in for Al ahli's fans side , the game was great because Alahli played really well but Al Iti7ad scored a goal in the second half and the excitement of the game lasted until the extra time when Al ahli had a penalty kick at the minute 94 but surprisingly Nader Kara took the ball from Omar dawood and went to score it but he lost it as we all expected it, not because Kara isn't a good player but he is not into Al ahli at all , he have been playing for months now but he still didn't get into the team and I guess after loosing that crucial penalty kick yesterday he should consider hiding for a while because now he is the number one enemy for all Al ahli fans .
The best thing in the game was the fans , so much fun so many songs they are singing and lots and lots of fireworks , I just wish the game was at night so we could have seen all the fireworks clearly .

Blood Donation

When was the last time you donated blood ?
Ask this question to anyone in the street here , I am sure more than 90% will answer never , and those who did donate, it would be directed donation i.e for someone they already know , like if his cousin is having operation he would donate for him , it's a big shame that it's very very rare here in Libya for someone to go by his will just to donate blood , I can't see any excuse for any healthy adult not to make one day every 6 months just to give blood , may be you don't know that one or even half liter would be saving someone else's life , someone will need it to make him live, may be you don't know that you can give part of your life to someone else , yes it's life you are giving here , because if you know how many people die here just because of bleeding you will be shocked , Last year in our Obestatrics course one patient was complaining how she suffered so much and she almost died because of bleeding and there was no blood to replace for her as fast as she needed , I asked her one question to stop her from blaming the hospital and the doctors and everyone else , Did you ever donate blood , she said no putting her head down , did your husband ever donate blood , she said we are married for 20 years now and he never did , I asked her again then from where the blood would come to you if almost all the Libyans like you , yes this woman could be anyone , it could be you , donate today because you have no excuse , and even if you didn't needed blood ever , someone else would need it for sure.
I donated before and I noticed something really different in the donation room from all the other areas in the hospital , you don't know how staff would be so pleased to see you and to serve you the minute they know you are here for donation , go to Alkhadra hospital on emergency entrance ask for the blood donation room and they will show you and help you out , and the same as in TMC ( Tripoli Medical Center ) go to the emergency entrance and ask in the reception where you can donate blood , they will take a blood sample from you first for viral screening HIV , hepatitis B and C , and to measure your hemoglobin level , the results will come the next day then when you come inshallah everything will be good and they measure your blood pressure and then you can give the liquid of life , you don't know how happy you will feel after that .

الطابور
امس فى الليل جانى كابوس نوضنى من النوم , حلمت انى واقف فى طابور و الطابور هذا مستقيم و مافيش حد يقص ولا ينقز لغاية ما وصل فيا الدور نوضت مخلوع , تعوذت من الشيطان لقيت روحى فى السرير ففرحت الحمد لله , معقول فى ليبيا وطابور مستقيم أعوذ بالله من الشيطان الرجيم .
انا ما فيش مرة ولو مرة واحدة وقفت فى اى طابورفيه أكثر من ثلاثة أشخاص فى ليبيا إلا وصارت فيه مشكلة ضرورى ما يجى واحد يقص و يلخبط الدنيا كلها , هذا لو فيه طابور اصلا لان الغالبية عبارة عن كدس من الناس , هذا النظام الجديد اللى حنى اخترعناه , تكدسو والصحيح فى الجسم او الوجه يتسقد فى الاول , خلي نعطيكم بعض الامثلة .
فى السوبر ماركت واقفين عادى فى الطابور جى واحد شاد باكو حليب قص قدامى ومد الفلوس , نفخت نفخة لين حسيت النار بتطلع من خشمى و فنصت فيه شبحلى ومش عاجبه اللى صاير , قلتله غير استأذن راهو بنادمية اللى قدامك , حتى صاحب المحل معجبتاش دوتي , قال : معليشى عنده باكو حليب واحد بس , قلتله كانا غير يستأذن نخلصهوله حتى انى الباكو , علاش معندناش احترام ,مش عارف علاش .
فى أى مقهى نطلب اى حاجة , زوز طابونة ,بيتسا,نص بالدحى اى حاجة المهم نقعد نستنى ويجى واحد بعدى وياخد نفس الحاجة اللي طلبتها قبلى, علاش عندنا الواسطة فى كل شئ, مش عارف علاش.
و مرة في مطعم الكباب متاع راس حسن واقفين فى الطابور زى الناس واحد لابيه لا عليه نادى ولده الصغير و قاله هاك الفلوس وبرا قص القدام فى الطابور وخود 6 كباب , توا مش الواحد يمزطله راسه على الحيط هذا ولا شنى, يعلم فى الولد القصان من توا بكري بكري باش ما يحصلش حصلته هوا توا .
فى الجوازات فى المطار انت واقف فى الطابور ستين واحد يقص قدامك ويحساب روحه دار حاجة كبيرة لما يختم الجواز قبل الناس , انا ديما نختم اخر .واحد وننزل نلقى شنطتي تستنى فيا واللى قصو كلهم قاعدين يستنو , علاش حنى فالحين فى التفشيك على بعض, مش عارف علاش .
فى أى جنازة انت واقف وتضرب فيك الشمس وتبى تعزي وتحصل الاجر تلقى ناس جاية باش بتورى وجهها وخلاص يتداعكو وينقزو فى نص الطابور, علاش تعب فى روحك يا بنادم مش عارف علاش.
فى الطريق لما تصيير زحمة تلقى السيارات قريب تركب فوق بعضها كل واحد يبي يوصل الاول والقصان على مفاصله من اليمين واليسار, الناس شايطة فيها النار , مش عارف علاش .
في الشيل ( محطة البنزين) انت واقف فى طابور السيارات يجى واحد بقمة الوقاحة والبرادة وصحة الوجه يجي بسيارته من قدام ويوخرها للماكينة , انا مرة حضرت عركة فى الشيل على خاطر القصان هذا , متاع الشيل قال للراجل حول سيارتك برا شد الطابور . الراجل سكر راسه يبى يعبى, متاع الشيل سخن و قاله تى والله ما تحولهاش الا لما نصب عليها بنزينة , ونتشلك فيها النار هنى, أحلى حاجة فى ليبيا تصكير الراس مش عارف علاش.
البنات والنساويين مرة مرة يفلفزولى فى رقبتي حتي هما ,تجي تقص قدام الطابور ببرودة وعادي وتاخد حاجتها قبل الكل من غير إحم ولا تستور تقول فايتة على صفة لوح مش ناس واقفة تستني , يأنسة يا أبلة يا أخت يا أمى يا هووه غير علاش القصان بس , مش عارف علاش .
خاطرى نعرف شني مشكلتنا مع الطابور ,علاش مفيش صبر , علاش الاستعجال , الكل يفكر غير نسقد روحى بس و طز فى الباقى.
ستالين ولا مش عارف منى زمان لما شد روسيا كان مداير قناصين فى اماكن الطوابير و اى واحد غير يمد زومته بس ( يطلع وجهه) من الطابور يصلفقوه برصاصة فى جبهته على طول , ياريت تطبق الفكره هذى هنى باش الليبيين كلهم تبدا جبهتهم منقوبة والواحد يوقف في طابور زي الناس .
يلا, شكلى توا بنمشي نشد طابور باش نمحي اثار الكابوس اللى شفته ونولي للواقع .

والحكاية مش خرافة لانه بجديات اول ما يشد المفتاح فى ايده يهدا وتمشى منه نوبة الصرع
لكن فيه تفسير لكل هذا وهو انه ما بين لقوله مفتاح ذكر تكون نوبة الصرع قريب تكمل و اول ما يعطوه المفتاح يهدا , يعنى المفتاح زايد ناقص والحكاية كلها زقاطة

so is there a response to a Emergency number calls , I don't know because I've never tried it and I don't know anyone who tried it , so in the end every one should know the Basic Life Support steps in the CPR so you can be ready at anytime and to know what to do if you witness someone went into cardiac arrest .

Libyan celebrations
Yesterdya after buying the book , I was going to my car and I heard sounds of mucic comming from Alwadi street , sounds of zukra and noba so I went down there but I didnt know what was the celebration , I guess it was wedding (zafa) , I took couple of pics and enjoyed the crowd for few minutes then left .
